Our community therapy programme is also open to young people who are able to continue living at home but who may benefit from accessing a comprehensive therapeutic pathway. We offer a limited number of specialist therapy packages to young people living locally to Northampton and South London. These pathways can offer the full therapy programme alongside all the supportive elements of the service including Therapeutic Care Advisor support, professional liaison, Care Programme Approach reviews and outcomes monitoring depending on the requirement for these additional services.
Transitions can be a vulnerable time for young people with mental health needs and we recognise at Aspire that it is important not to cut contact with those we have cared for suddenly and wherever possible to plan in advance for their gradual transition. Where further support and continued therapy is required, young people can step down from our residential pathway to our community pathway and continue receiving the therapy and even Therapeutic Care Advisor support in their new home. We can work with their professional team to tailor this transitions package around the individual needs of the young person.
